We know handling health challenges can be difficult. Our care management team is here to help. Care managers partner with you to help navigate complex health needs.

They can assist with appointments, answer health questions, support post-hospital care and more. For eligible members, care management is available at no extra cost to you.

Care managers are trained to help coordinate your care and communicate with your doctors, specialists, and other providers. 

Care managers can help you with complex and chronic health conditions, so you get the care you need. They are here to support you throughout your health journey. They can:

Coordinate

  • Communicate with your doctors and caregivers.
  • Set up appointments. 
  • Keep track of prescriptions and set up home delivery for your medications.
  • Connect you with healthcare providers, services, and resources.
  • Assist with the process to get medical equipment, such as blood sugar test strips and blood pressure cuffs.
  • Help arrange follow-up care after a hospital stay and review medications.

Educate      

  • Offer information and resources on your health conditions.
  • Answer questions about your health, medications, and needs.

Support

  • Create a plan so you can work toward your health goals.
  • Provide tips to improve your overall health.
  • Locate community resources, from support groups to food banks.
  • Make sure you get the care you need.

We know caregivers are an important advocate for members. Care managers can work with your approved caregivers to help you get the care you need. Our team can partner with your caregiver to coordinate care, answer health questions, and more.
